By signing this document I hereby give representatives of Creepin’ Kitties (CK) and full permission to trap the cats/kittens located at the above stated location. I take full responsibility to the whereabouts of the traps and the traps themselves should they be stolen. I will tend to them regularly, so they are not left unattended for any period of time. I take responsibility for any issues that may arise from trapping on this property and will exempt CK from any liability.

I will surrender the cat and kittens to CK upon trapping for care. I understand that the adult cats may be returned to the area due to their feral nature after they are spayed/neutered, given a rabies vaccine, distemper vaccine, treated for fleas/ticks/internal parasites, and receive an ear tip on their left ear. I understand that CK does not permanently remove the cats from the area they are familiar with. There may be circumstances where this is not the case, but most cases require cats to return to the area they are trapped at.

CK focuses on TNR, trap, neuter, release for community cats. Non-feral cats and kittens may be adopted out through CK and an application is necessary for anyone interested in adopting them. The adoption fee is $50 per cat/kitten.
