Dear Adopter:

We are thrilled that you are adopting with Goldens 901 and excited about the wonderful future ahead for you and your new family member.  Please complete and sign the below contract.  If you have any questions, please feel free to reach out to your adoption representative.

Adoption fees are outlined on our website and will be communicated to you by the adoption representative.  These can be paid via check or with credit card. If paying by check, please make checks payable to “Goldens 901,” and send to the following address:

We would appreciate receipt of your adoption fee concurrent with the return of this Adoption Contract.   Upon receipt of this contract and the adoption fee, the adoption process is complete.  

Your dog is microchipped with a microchip manufactured by Pet Link and inserted by our veterinarian. Your adoption representative will provide the microchip number to you.  Please visit the Pet Link website and update microchip information with your contact information.  Because the microchip has not been registered, this process can be completed as follows:

•  Navigate to the Petlink website at www.petlink.net
•  Click on “Register a Pet”
•  Enter the microchip number provided to you
•  Follow the prompts to register your pet

Please note, in some instances your new dog may have been previously microchipped before it was rescued.  In those instances the process to update information will be different than the process above.  Your adoption representative will assist you in those circumstances.

The Pet Hospitals, with seven locations in the Memphis area, has been providing quality veterinary care for your new pet during his/her stay with Goldens 901.  Please reach out The Pet Hospital’s main location located in Collierville, TN, (901)853-7330 to obtain veterinary records and, if you wish to continue with The Pet Hospitals, transfer care to you.   If there are any long-term medical needs that will need to be tended to on an ongoing basis, you will be made aware during your adoption discussions.

Prior to your adoption, your new pet was cared for and loved by one of our generous volunteer foster parents.  The foster fed him/her (once/twice) daily.  Your adoption representative will provide you the name of the current food being used along with the feeding schedule and any pertinent information that you may need concerning their care.  There are several quality foods available and should you desire to switch to a different food we suggest you do so gradually over a period of several days and/or include additives like pumpkin or probiotics as recommended by your vet, to minimize gastrointestinal disruptions during the transition to the new food.

I agree this dog is being adopted as a family pet and will be given extensive house privileges, i.e., sleep inside the house and be allowed freedom of the house when the family is home.

I agree the dog will live primarily indoors and will NOT be left outside, even in a fenced yard, for extended periods unsupervised.

I agree to never tie the dog to a rope or chain, or let the dog run loose in the neighborhood.

I agree to provide the animal clean water, high-quality dog food, adequate shelter, affection, exercise, and kind treatment.

I agree to take the animal to a veterinarian as needed for examinations, vaccinations, heartworm treatment and other medical issues that may arise.

I agree to notify Goldens 901 if I decide at any time I cannot keep the dog. I agree I will NOT rehome, surrender to a shelter, or abandon the dog without first making every attempt to contact Goldens 901.

General Release
I agree to adopt the dog from Goldens 901 under the terms of this agreement. I acknowledge dogs are unpredictable and that Goldens 901, its past, current, and future board members, fosters, volunteers, agents, donors and any partners, associates, attorneys, and other related people or entities (collectively “Released Parties”) cannot and have not guaranteed the health, age, temperament, breed, behavior, training, or any other conditions or characteristics of the dog.

I understand that the dog may over time exhibit traits that may or may not be apparent at adoption and it is still my desire to adopt the dog under the terms of this agreement.

I have met the dog and spent adequate time with the dog to make my own informed decision to adopt the dog and I am not relying solely upon any statements or representations made by the Released Parties in making my decision to adopt the dog.

Once the dog is within my possession, I release each and all of the Released Parties from all claims or liability relating in any way to the dog once he/she is in my possession. To the extent any such claims or liability should arise, I agree to defend, indemnify, and hold harmless the Released Parties for any such liability or claims.
