Please complete this application to submit your nomination. Membership in SOFT is required to both nominate a recipient and receive an annual award. Self-nominations will not be accepted.

Nominations require:

• The selection of a nomination category
• Rationale for nomination, not to exceed 1,000 words
• A resume, CV or professional background information on the nominee

Optional information that may be included:

• Up to two letters of support for the nomination, not to exceed 1,000 words each. Letters exceeding 1,000 words will not be considered. Letters of support do not need to be written by SOFT members.

Please provide background inforamtion as to how the applicant meets the criteria. This can be typed or pasted in the box below, or a document can be uploaded. Please also upload a resume, CV or a document describing the professional background of the nominee.
