Please list any medications your camper will be taking at camp. Any changes to this list MUST be communicated to a stake leader before camp. ALL MEDICATIONS MUST BE IN PROPERLY MARKED MEDICATION CONTAINERS AND CHECKED INTO THE WARD CAMP LEADER AT THE START OF CAMP!
We, the undersigned, as the parents/legal gardians/myself of blanks hereby give permission to attend and participate in the supervised Lawton Stake Young Womens Camp program of the Church of Jesus Christ of Latter-day Saints at Salyer Lake campground.
We authorize the camp nurse, camp director, qualified members of the camp staff, or driver of a vehicle to obtain first aid or other care to our said camper/myself in the event of an accident or illness.
We further authorize said persons to take any provisions for medical and/or surgical care for our camper/myself, including anesthesia, which may be deemed necessary or advisable by any licensed physician. We assume and shall be responsible for all medical costs and expenses in connection with the care and control of our said daughter except in so far as there is applicable insurance covering the same.
In the event of an incident requiring medical attention, we expect that every effort will be made to notify myself or the above named emergency contact.
This authorization shall remain effective until the above-mentioned minor returns home from Young Womens Camp, unless revoked sooner in writing.

2025 Young Womens Camp Contract

PLEASE READ THIS THOROUGHLY AND SIGN IT APPROPRIATELY!
1. Each Young Woman will follow the rules and standards of the church, including, but not limited to:

-NO smoking/vaping

-NO alcoholic beverages

-NO profanity

-NO drugs and/or marijuana

2. Each Young Woman must wear modest and functional clothing at camp. Shoes will be worn at all times.

-Tennis shoes are needed for hiking and walking, but sandals and flip flops are ok for non-athletic activities.

-Modest one-piece or long tankinis (that cover the belly) swimsuits only.

-Immodest clothing is any clothing that is tight, sheer, or revealing in any other manner. Young Women should avoid short shorts and short skirts, shirts that do not cover the stomach, and clothing that does not cover the shoulders or is low-cut in the front or the back.

3. NO hurtful or harmful pranks

-Do not touch or disturb anyone's belongings in any way.

-Do not hurt anyone's feelings in any way. NO BULLYING.

-Do not enter anyone's cabin/dorm without permission from someone inside that cabin/dorm.

4. YW will not damage or deface ANY property.

-NO writing on cabin/facility walls.

-Leave camp in better shape than you found it.

-There will be a charge for any damage done to any of the camp property.

5. YW must have a buddy at all times and must never leave the group or camp without permission. This is important for your safety. Sneaking out or leaving the camp area will result in dismissal from camp.

6. Reasonable, safe, and appropriate behavior is required at all times. Young Womens Camp is NOT the place for romantic behavior, nomatter your sexual orientation.

7. Electronic devices are discouraged. Cell phones can be used for scriptures, but the coverage is spotty, so unless it is downloaded to the phone, it may not be available.
I,blanks , understand and agree to abide by the Stake Young Womens Camp rules. I understand that if I make the choice to break these rules, I will be held accountable. I understand that there will be consequences for breaking the camp rules which may include notifying my parents and/or having them remove me from camp.
