COURSE ENROLLMENT (Please read)

• The schedule may change depending on the demand for certain classes.
• For Term 1/2024 the Group Lesson consits of 11-13 lessons (no class during public holidays).
• Any make up class due to weather or sick (Doctor's Note is required) are scheduled seperately. Please arrange with your swim coach for lesson make up date.
• There will be NO REFUND for any unattanded makeup lessons. 
• Class confirmation will be sent to your e-mail.

REGULATIONS - Please read:

1. Swimming lessons are for Club members and their guests only.
2. All members/guests must submit application form prior to commencing their first lesson.
3. Applications must be completed through JOTFORMS only.
4. All applications for coaching will be processed by the club’s appointed coaches.
5. In case of bad weather, members are required to contact the Gym Reception on 2657 8292, one hour prior to the lesson, to enquire if the lesson is canceled.
6. There will be no lessons in the event of typhoon signal No. 8 or Red/Black Rain Storm warning being hoisted and make up class will be offered.
7. Children under 10 should be accompanied by an adult and all children should be collected promptly after the lesson has finished to ensure their safety.
8. For Term 1, there are 11 to 13 lessons in total and all students must attend all the scheduled lessons. 
9. If a student does not show up for a scheduled lesson, full lesson fee will be charged and there will be NO REFUND for all other reasons except for weather. 
10. If student is sick and unable to attend lessons, a Doctor's Note is required in order to arrange for make up lesson and must be presented to us within 48 hours. 
11. There are no group lessons during Public Holidays.
12. For Club members, payment will be debited to their membership account once the lessons are confirmed.
13. For non-members, full payment must be made after receiving lessons confirmation by bank transfer or by cheque. A 10% surcharge will apply to the total course fee. Bank transfer details: HSBC 808-271852-001 or if paid by cheque, made payable to Hong Lok Yuen Country Club Ltd.
14. If payment is made by bank transfer, please e-mail to us the bank transaction receipt as payment proof to recreation@cchly.com and for ease of referance. Payment can also be made through Member's account.

By submitting this form, I agree that the relevant coaching fees will be charged to my Club account upon confirmation of my/my child's first lesson and understand the Cancellation/No Show policy as stated above.

I also acknowledge that I/my child am/is taking part in this activity at my/their own risk, and that the Club and its staff/contractors are not liable for any injury/accident to me/my child, however caused.
