PRIME DEFENSE FIREARMS TRAINING LLC.

Release of Liability, Waiver of Claims, Assumption of Risks, and Indemnity Agreement

In consideration for being permitted by Prime Defense Firearms Training LLC, and its owners Shawn Crowl and Jerry Crowl, (collectively referred to as "Released Parties") to participate in firearms training activities, the undersigned, hereby acknowledges and agrees as follows:

1.Assumption of Risks: I understand that participating in firearms training involves inherent risks, including but not limited to, the risk of injury or death.

2. Release and Waiver: I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, or causes of action for any loss, damage, or injury, including death, that may be sustained by me while participating in firearms training.

3. Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in firearms training.

4.Medical Treatment: In the event of an emergency, | authorize the Released Parties to secure medical treatment for me.

5. Compliance with Rules: I agree to abide by all rules and instructions provided by the Released Parties during the firearms training.
