The primary objectiveof this tool is to pinpoint and document potential risks associated with eachtransition to or from the provider. This encompasses not only typicaltransitions but also temporary shifts that may occur in response to specificrisks faced by the participant, such as instances necessitating hospitalizationdue to healthcare concerns. The aim is to comprehensively identify, document,and respond to these risks to ensure the participant's safety and well-beingthroughout the transition process.
