  • 8th Annual Play It- 4 on 4 Team Registration Form

  • The event will take place on Saturday, June 1, 2024, behind École Dansereau Meadows School. Games are anticipated to be scheduled between 8 AM to 4 PM, with a minimum of three games per team.

    • All players must supply their own equipment, including gloves, helmet with face shield, and wiffle stick or hockey stick; shin pads are recommended.  No goalie equipment will be provided. 
    • You will be required to register as a team, 5 to 7 players is ideal, but up to 8 are allowed. Registration will be done based on birth year, not grade level, for kids born between 2010-2018. Teams will be categorized into divisions, based on registration numbers and competition level. Teams should have players close in age, but same birth year is not required, however older players may affect team placement.
    • Every participant is required to sign a waiver form. Once your team is registered and payment has been received, you will receive a link to the online form.  
    • A minimum of three teams is needed per age category. In the event there are not enough teams registered, registration fees will be refunded in full. 
    • Pizza lunch will be provided for players. There will also be a concession, cash only. 
    • Prizes awarded to the team who demonstrates the best Team Spirit!
  • Team Manager Requirements

  • The following requirements must be met by the Team Manager: 

    • complete the registration form, collect individual payments from players, and submit one team payment via etransfer (payment details below).
    • direct all communication from the street hockey planning committee to the players/families. This includes the participant waiver form, tournament rules, and schedules. 
    • On the day of the event, you will be responsible for checking your team in and ensuring that they are at the correct rink at the correct time. You will be expected to be present throughout the duration of the event. 
    • You will be responsible for organizing your team for the provided pizza lunch. 
    • Each team will need to provide an adult score keeper for each of their games. The team manager will be responsible for assigning this task to parents. 
  • Payment

  • The registration fee is $275 per team. The payment is to be made via etransfer to edms.parentassociation@gmail.com. Please use the password "dansereau" (all lowercase letters). In the notes please put the Team Manager name. Only send one etransfer per team.  Registration is not considered complete until payment has been received. 

  • Getting Involved

  • This event is a fundraiser organized by the EDMS Parents Association and is our largest fundraiser of the year! We'd love to have you on our team.

  • Respectful, sportsmanlike conduct is expected from all players, parents and spectators. The EDMS Parent Association and tournament committee reserve the right to remove individuals from the tournament who are not meeting these expectations. We also ask that your team works together to clean up after themselves throughout the day, showing respect to the school and school grounds. Please remember this is a fun community tournament fundraiser for kids and youth to enjoy.
