Spanish Fork Veteran Banners on Main Street

This project offers a meaningful opportunity to honor veterans—both living and deceased—who have resided in Spanish Fork or within the 84660 zip code. A part of the proceeds will go to support the Spanish Fork Historic Preservation Commission.

Veterans will be displayed on one side of a two-sided banner along Main Street. Banners will be displayed on and around Memorial Day, Veterans Day, and in the Fiesta Day parade in July (if volunteers or family members carry them). To have your banner posted for Veterans Day 2025, orders must be placed by Noon on September 10th , 2025

Please make sure to know/have these items before filling out this form: 

1. Branch - Marine Corp, Army, Navy, Air Force, Coast Guard, National Guard, and Space Force Military
2. Rank ex. Private, Private First Class, Corporal, Airman, Specialist, Senior Airman, Petty Officer, Staff Sergeant, Lieutenant, and many more
3. War or Location of Service
4. Years they served or decade they served in
5. High-resolution photo of your veteran in uniform - Photos taken off of social media or the Internet are not acceptable. Futher details provided below.
6. Designation (Optional) Killed in Action (KIA), Missing in Action (MIA), or Prisoner of War (POW)
7. Biography (Optional) Up to 300 word biography for a story map on the City’s website This allows individuals to discover more about your veteran and locate their position on Main Street
8. Photos (Optional) Up to 2 additional high resolution photos 
9. Cost $250 for one side of a banner. Banners will be displayed for 3 years
10. If the Veteran is living please submit an affidavit that they give us permission to use their picture and information.

Banner Photo

This is the photo that will display on the banner. The photo needs to be a high-resolution photo of your veteran in uniform.

The banner photo quality should be at least 5 mb or higher. Do not send photos taken off the internet or social media. When scanning the photo, it is recommended for a 3”x5” photo or larger to scan at a minimum of 300 dpi or for smaller photo a minimum of 600 dpi. It is recommended to scan yearbook photos at 2400 dpi. If you need help scanning your photos, email history@spanishfork.org.

Your veteran's biography, along with extra photos, can be submitted for inclusion in a "story map" on the City's website. This website allows individuals to discover more about your veteran and locate your veteran's position on Main Street.

The deadline for submitting content for the story map is October 12. If you choose not to use the provided form, kindly email your veteran's biography to history@spanishfork.org (subject to review and optional).

Name and Likeness. By signing, including electronic signature, you consent to use by Spanish Fork City ("City") of your deceased relative's ("Veteran") name, nickname, image, likeness, initials, reputation, autograph, biographical data, and likeness ("Name and Likeness") in connection with the display, exhibition, distribution, advertising, publicity, and promotion of the Veteran in all media, whether now known or hereafter devised, perpetually, and without restriction.

Proceeds. You acknowledge that all proceeds from this agreement shall be the property of the City or the City's Historic Preservation Commission.

Warranty of Authority; Hold Harmless, Defense, and Indemnity. You represent and warrant that you have authority to grant the City permission to use, as described herein, the Veteran's Name and Likeness. You agree to indemnify, defend, and hold harmless and release the City, its officers, employees, agents, and contractors from any and all claims, liabilities, damages, expenses, and costs arising out of or related to the use of the Veteran's Name and Likeness, including but not limited to any claims for defamation, invasion of privacy, or infringement of intellectual property rights, whether such claims are brought by a relative of the Veteran or otherwise. In case of any dispute of authority or any claim brought in connection with the use of the Veteran's Name and Likeness, the City may choose to discontinue its use, that being the sole remedy.

Ownership. The banners, signs, photos, and all other media that bear the Veteran's Name and Likeness shall be owned by the City in perpetuity.

Entire Agreement. This Agreement constitutes the entire understanding between the parties and supersedes all prior agreements, whether written or oral, relating to the subject matter hereof.

Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the state of Utah, without regard to its conflicts of law principles. I understand that the banner will be kept in storage by Spanish Fork City when not in use and will not be returned.
