Hello, my name is Chumby (they/them) I currently tattoo in Monterey Park, CA. I'm excited that you want a tattoo from me! I book customs that fit in my style range and flash designs. For tattoos, I charge a flat rate. I book by appointment only. Keep in mind that this is a REQUEST form and not officially booking your appointment.
If you would like to book with another person {friend(s), family member(s), or partner(s)} on the same day as your appointment please ask them to submit a separate booking form from yours and include that they want to book the same day as you! Thank you!
If you are traveling from out of town please let me know the specific dates you will be in town so I can try my best to work with your travel plans

Pricing & Customs
Pricing is based on size, color, & placement
All premade designs have a minimum size requirement.
Custom minimum is $200.
Most premade designs range from $200-$500
For Custom designs, a sketch of the design is sent out a day before the appointment with room for changes prior to the official appointment date and on the day of the appointment the final design is shown. If there are more changes that want to be made on the day of the appointment we will most likely need to reschedule the appointment to allow more time for the tattoo.
Deposit
A $80 non-refundable deposit is required to book and secure an appointment. It will be deducted from the overall cost. This deposit can apply to one reschedule with a 24 hr notice. No call no shows/ same day cancellation forfeits deposit.
