We will attempt to reasonably answer any financial concerns prior to the delivery of any performed procedures, services or products you may request.
We are a private clinic therefore we take no insurance. however we can provide you with a super bill to submit to your insurance company for possible reimbursement. It is your responsibility to submit the form. Any prescription provided may be filled at any pharmacy.
Our practitioner consultation begins at US $50-$399 and may be more depending on the services and time provided to you. If you are seeing the Nurse Practitioner for a specified procedure or follow-up appointment this fee may not apply.
Please be advised that payment is expected prior to provider contact and all services and products are final, non-negotiable and non-refundable after tender. If you have purchased a package and do not wish to continue sessions, any refunds for unused services will be done at the a la carte pricing, NOT THE PACKAGE PRICING.
We expect payment at time of service. We accept all other major credit cards including American Express, MasterCard, Visa and FSA/HSA Cards. Checks are prohibited, but you may pay with US dollars, Cashapp or Venmo. You are advised any chargeback or dispute with the bank of card holder regarding any paid and signed transaction will be invalidated by signing this document.
The Institute for Optimal Healing & Aesthetics is committed to providing all of our patients with exceptional care. When a patient cancels without giving enough notice, they prevent another patient from being seen. Please call or text us at (954 802 9835) to notify us of any changes or cancellations within 12 hours to avoid the fee of $49. If more than 2 no-shows occur, you will have to pay $100 to be seen.
Any question regarding price is incumbent upon the patient to question prior to the delivery of said service or product prior to being performed, recommended or sold. Treatment plans will be provided with breakdown of pricing and will be required to be signed prior to being serviced.
Any prepaid services purchased in advance, must be utilized within 1 year of purchase. Unused services will be refunded at our discretion. Services already in progress will then be repriced without the packaged discount prior to the refund.
We will honor the pricing on a quote through the expiration date on that quote. Otherwise, our prices are subject to change at any time.
We aim at providing you with all the knowledge you need to make an informed choice before starting treatment. Because aesthetic medicine is not an exact science, unknown reactions from your body are possible. We'll give you the greatest treatment possible while always keeping your aesthetic objectives in mind.
