- Completed short essay answers in the application.
  Provide up-to-date transcript from JMCS showing completion of credits for graduation.
- Two letters of recommendation
- If applying for a post-secondary education award, provide a copy of your Student Aid Report (SAR) or CADAA award letter
- Provide a recent photograph of yourself
