Beats & Balance Trap & R&B Yoga Class Waiver

I, {name}, hereby acknowledge and agree to the following terms and conditions in consideration of being allowed to participate in the yoga class offered during the Beats & Balance Trap & R&B Yoga Friday, April 11, 2025, at The Yoga Lab 3724 Mayfield, Severance Center Cleveland Heights, OH 44118.

Assumption of Risk:

I understand and acknowledge that participation in any physical activity, including yoga, involves inherent risks and the potential for injury. I am voluntarily participating in this activity with the knowledge of the risks involved and hereby assume full responsibility for any and all risks, injuries, or damages that may occur as a result of my participation.

Health and Fitness:

I affirm that I am in good health and physical condition to participate in the low-impact yoga class. I acknowledge that it is my responsibility to consult with a medical professional prior to participating if I have any concerns about my health or ability to engage in physical activities.

Exclusion for Pregnant Participants:

I understand that if I am 36 weeks or more pregnant, I am excluded from participating in the yoga class. I will inform the instructor of my pregnancy status and will refrain from joining the class if I am beyond the specified pregnancy term.

Waiver of Liability:

I, for myself and on behalf of my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and hold harmless the Pregnant with Possibilities Resource Center, The Yoga Lab, their respective officers, directors, employees, volunteers, and representatives from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during my participation in the low-impact yoga class.

Photographic Release: I grant permission for the use of any photographs, video recordings, or other media taken during the yoga class for promotional, educational, or other related purposes by the Pregnant with Possibilities Resource Center and The Yoga Lab.

Photo Release

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used for marketing and promotional use.
