Conditions for Receipt of Merit-Based Grant-in-Aid

I understand that to qualify for this financial aid, I must
- meet the admission requirements of John Melvin University
- meet and maintain the eligibility requirements for financial aid established by the USCAA and John Melvin University
- abide by the Athletic Department’s academic standards program as set forth in the student-athlete handbook;
Reduction or Cancellation of Grant-in-Aid

I am aware that the amount of this aid may be immediately reduced or canceled during the term of this award if:
- I do not complete this agreement and all application requirements within 14 days of this agreement date.
- I become academically ineligible (for example, by carrying less than 12 applicable semester credits);
- I give false information on my application, letter of intent or, this financial aid agreement;
- I engage in serious misconduct that brings disciplinary action from JMU
- I voluntarily withdraw from the sport for personal reasons prior to the first competition in my sport; or
- I do not meet the “conditions for receipt of grant-in-aid” outlined above.
I am also aware that this aid must be reduced or canceled if:
- I sign a professional sports contract for this sport;
- I accept money for playing in an athletic contest;
- I agree to be represented by an agent; or
- I receive other aid that applies to my individual limit.
MORALS CLAUSE: Any JMU student shall not commit any act or do anything which might reasonably be considered: (i) to be immoral, deceptive, scandalous, or obscene; or (ii) to injure, tarnish, damage, or otherwise negatively affect the community and/or the reputation and goodwill associated with John Melvin University. If the student is accused of any act involving moral or ethical issues, dishonestly, theft or misappropriation, under any law, or any act which casts an unfavorable light upon its association with the community and/or the University or the student is accused of performing or committing any act which could adversely impact the University programs, services, or reputation, JMU shall have the right to terminate this contract upon fifteen (15) days written notice specifying the reason, within which period the student may cure such offense. The determination of whether and to what extent the offense is cured shall be made by the University at its sole discretion.

Name, Image, and Likeness Release

For good and valuable consideration, I do hereby agree as follows:
1. USE OF NAME, IMAGE, & LIKENESS. I hereby grant to JOHN MELVIN UNIVERSITY and its affiliates and assigns (“JMU”) the right and permission to use in perpetuity, my name, likeness, image, voice, recorded voice, appearance, biographical information, statements, performance and/or testimonial(s) (collectively, “Appearance”) in any manner and in any media now known or later developed, throughout the world, at any time, for the purpose of advertising and publicizing the JMU’s products and services, without review, permission or compensation of any amount or kind whatsoever. JM University shall have complete ownership of any recording, product, copy, presentation or other material or file containing or featuring my Appearance (“Product”), including copyright interests, and I acknowledge I have no interest or ownership in the Product (or any portion thereof) or its copyright. This grant includes without limitation the right for JMU to edit, abridge, augment, title, or create a compilation from my Appearance in whole or part as JMU may elect in its sole discretion.

2. RELEASE OF LIABILITY. I shall not hold JMU responsible for any liability
resulting from the lawful use of my Appearance in accordance with the terms of this agreement.

3. NO OBLIGATION. I understand that JMU has the right, but not the obligation to
use my Appearance.

4. NO CONFLICTS. I confirm that I have the right to enter into this Agreement, that
I am not restricted by any commitments to third parties, and that JMU has no
financial commitment or obligations to me as a result of this Agreement. I will not
authorize or permit the use of my name, voice, photograph, likeness, or testimonial in
connection with the advertising or promotion of any product or service competitive to
or incompatible with JMU’s products or services for four years after the date of this
agreement.

I agree that this agreement shall be construed in accordance with the laws of the state of Louisiana, as contracts made in Louisiana.
