In consideration of the information below having been supplied, the dog has been donated to Midlands Cleft Rescue. The parties of this agreement hereby agree as follows;

I confirm that I have the authority to enter into this agreement and should my authority be subsequently questioned by Midlands Cleft Rescue or any third party, I agree to take full responsibility for entering into this agreement and if necessary compensate any third party

On signing this agreement the Dog described is accepted by M.C.R with ownership and all rights to the dog entirely passing to M.C.R

The person signing the dog over to M.C.R has no right to receive the dog back or exercise any other rights in respect of the dog.

I give my permission for the vet detailed as given here to be contacted for a medical history if necessary

I have read, understood and agree to the above conditions

The information I have given in this form is true and correct to the best of my knowledge

I am over 18 years of age.
- Any donations that can be made to the rescue would be extremely helpful, this all goes to the aftercare of the puppies being surrendered, including high
