The deadline for completion of this form is May 3, 2024 Fellows are matched with a mentor as part of their Institute year. This questionnaire helps staff match Fellows to mentors who can support your specified areas of interest, growth, or stated goals. Potential mentors are typically selected from among the Member Network, and other partners and supporters. Our goal is to connect you with a mentor who can provide meaningful guidance. Mentors and mentees ideally meet (virtually/phone) at least four times over the course of the mentorship from June to December, but the level of frequency is determined by each pair.
