Application for CazArts Kids/Seniors/Disabled Scholarship

This form is an application to receive a scholarship towards payment for CazArts classes at Carpenter's Barn.
NOTE: This form is NOT applicable to the Kids Summer Music Theater class. For that class, please use the following: <LINK>.

Scholarships are available for kids and for Madison County residents who are disabled and/or 65 and over. These scholarships are limited to one class/workshop per family per session.

This scholarship program is "pay what you can." Please indicate below which class you are applying for and if you are able to contribute toward your class fee. If you can't contribute that is fine too, just select "I need a full scholarship."
