The Nomination Form requires the following information: 

1. Part A: Nominee Contact Information
2. Part B: Nominator Contact Information
3. Documents Supporting the Nomination (word,pdf documents)
   • A letter of reasons for nomination
   • A CV/Resume of the nominee
   • 2 other supporting documents (optional)

Please be noted that you can save and return to your submission.  To do this, click on the save button at the bottom of the form (visible on the pages where you can fill in content).  You will also receive an email with a link to return to the form.
