The safety of all participants is our first priority. We reserve the right to refuse further participation and will refund all or a portion of the registration fee at our discretion. By signing below, you certify that all information provided is accurate and that you hold harmless Lynzie Bacchus and Tractor supply. instructors, speakers, volunteers and assistants for any incident that occurs before, during or after the class or event.

Goals of attending class *

You agree that the $5 NON-REUNDABLE deposit is non refundable unless the instructor cancels the class.

You will pay the balance of $120 at the start of the class. Once you attend the first class if you decide to cancel on your accord all fees are NON-REFUNDABLE!

Bring copy of dogs Rabies to the first day of class.

If puppy is too young bring copy of vaccines. Puppy wil need min 2 sets of vaccines to attend class.
