CONSENT FOR PARTICIPATION IN ACTIVITIES | As the parent or legal guardian of the above-named participant, I hereby give my consent for my child to participate in various events and activities organized by Deploy Conference 2024. I understand that it is my responsibility to review the details of each event and determine whether it is suitable for my child's participation.I authorize the conference staff, volunteers, and designated leaders to supervise and care for my child during the events, including providing necessary assistance, guidance, and support as needed.I release and discharge The Sending Church, Deploy Conference 2024, and its staff, volunteers, and affiliates from any liability for injuries, losses, or damages that may occur as a result of my child's participation in the events, except in cases of negligence or misconduct on the part of the church. I understand that photographs, videos, or audio recordings may be taken during the events for promotional or educational purposes. I consent to the use of my child's likeness in such materials without further notification or compensation.
CONSENT FOR MEDICAL TREATMENT | As the parent or legal guardian of the above-named participant, I hereby give my consent for emergency medical care in the case of an emergency. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent.
