This form is for you if you are a guest whom I have seen in the salon within the past 6 months. If you are a new guest, or you have not seen me in the salon within the past 6 months, please fill out the new guest appointment/ request at www.sarahcrews.com/newguest so we can get caught up on your hair history. I will be in touch within 24 hours of your request during my in-salon days, which are Monday & Tuesday.

Generally, I am able to get you into the salon within 2-3 weeks of appointment submission request, but guests with limited availability may have to wait up to 12 weeks to be seen.

I understand that there is a strict 48 hour cancellation policy and if I do cancel my appointment with less than 48 hours notice, the total balance of my appointment value will be due before booking another appointment.
