JOP Month-to-Month ULTIMATE MAX Reformer & Fitness Membership Terms:

1. WAIVER: Member represents and warrants that all facts stated in the New Clients Waiver of Liability submitted by Member are true and correct. The Waiver is incorporated herein by reference and made a part of this Agreement. New students must sign the New Client Waiver of Liability.
2. DURATION: Your JOP Membership is a 1-month (30 days) Membership starting on Date
3. MEMBERSHIP FEES: Under this option, an auto payment in the amount of $275 (+tax) per month will occur on the date specified above. Your JOP Membership fee will be paid via auto payment using the credit card on file in MindBody on the Enter Day date of each month. All payments to JOP are final and are not refundable. We reserve the right to change the price of membership/ classes. We will notify you by email (at least) 30 days before the price change. If you do not agree to the price change, you must notify Joy of Pilates to cancel and stop using the Services before the price change takes effect
4. COMPLETION: The JOP Membership can be terminated after 1 full month of Membership.To end your Membership you must notify the Front Desk in writing 10 days prior to the autopay datedesignated above.

5. UPGRADING MEMBERSHIP: If at any time - Member wants to change to 1-Year Ultimate or Unlimited Membership please notify the Front Desk. Months paid will be applied to 1-Year contract.

a. If you wish to cancel this contract within 3 business days of signing you may cancel it by delivering or mailing a written notice to JOP. The notice must say that you do not wish to be bound by the contract and must be delivered or mailed before midnight of the third business day after you sign this contract. The notice must be mailed to 2130 Grant Street, Bellingham, WA. If you cancel within the three days, JOP will return to you within thirty days all amounts you have paid.

b. You may cancel this contract on your disability with certification from your health care provided; and it is deemed canceled on your death or total disability;

c. If you move your permanent residence more than 25 miles from the studio or an affiliated studio with the same services and facilities, you may cancel this contract after one year from the date of signing; or, if at the time of signing your permanent residence was already more than 25 miles from the studio or an affiliated studio with the same services and facilities and you move your permanent resident an additional five miles from the studio or the affiliated studio, you may cancel this contact after one year from the date of signing.

d. If the contract extended for more than one year, upon thirty days’ written notice;

e. If the studio is permanently closed

7. MEMBERSHIPS CHANGES: Other than as set forth above, there is no option to cancel this contract.Please see options below regarding changing or transferring your membership.

a. GIFT: Gift the remainder of Membership.

c. CHANGE MEMBERSHIP TYPE: At any point during the Membership contract, members may switch their membership to a different membership that JOP offers.
ACCEPTANCE OF TERMS. As a Member, I understand that I am entitled to use the facility within the scope ofthe Membership that I have selected, and that I am obligated to pay my Membership fees regardless ofwhether or not I use the facility. I agree to promptly update Joy of Pilates with any change in my contactinformation (including address, telephone number or email address) or change in credit card information.

I certify that I have read the foregoing Membership Agreement, and that by signing below, I acknowledge that I understand and agree to be bound by all the terms and conditions hereof. I further acknowledge that a fully executed copy of this Membership Agreement has been provided to me
