DISCLOSURE
I hereby authorize Antecia Technologies (“Antecia”), agent for the owner of the rental space or the employer who requests it, to obtain the following information as part of my prequalification:
I hereby consent to a credit history check with identity verification which will include information about me, including prior bankruptcies, civil court proceedings, collection actions, negative banking items and other information declared by my creditors, and I hereby authorize any public or private institution to provide Antecia with information relating to my credit report.
I hereby consent to an identity check to verify my identity. This verification will be conducted using consumer databases, government records, identification documents I have provided (if necessary) and other municipal, state and federal records.
I hereby consent to a review of any publicly accessible websites, including but not limited to the internet, news feeds or articles from any source (electronic or otherwise). This review may include written articles, images and/or videos. I understand that these data sources will only be searched if they are accessible to the general public and are not protected by a website's privacy settings or if viewing them requires specific permissions.
I authorize Antecia Technologies to obtain information about myself and the actions with which I am or have been involved in any provincial or federal court, administrative tribunal, regulatory body, disciplinary committee or any legal authority or quasi-legal in Canada. In the event that I have lived in a country other than Canada, I further authorize an equivalent criminal record search in all countries where I have resided, within the limits permitted by the legislation of these countries.
I also authorize Technologies Antecia to disclose the results of its verifications to the owner of the rental space or to the employer who requests it.
WAIVER
Purpose of consent: Background check for the owner of the rental space or for the employer requesting it (The company)
I understand that the information obtained through this consent will be held strictly confidential by Antecia Technologies and/or the Company and will be held in accordance with their respective confidentiality policies. The information obtained will be used in accordance with, and only to, fulfill the purpose for which the authorization was signed. I hereby release any person providing information in connection with this authorization from any liability for the disclosure of the information to Antecia Technologies or the company.
I certify that the information I provided in this authorization is accurate. Before signing this authorization, I have informed myself of its content and its meaning and understand it completely. I also understand that the information will be stored in Canada.
Note: You can request access to your personal information, a request for correction of information or any other request by sending a request by email to Antecia Technologies at info@antecia.co, to the attention of the Privacy Protection Officer. Antecia Technologies will respond to your request within (30) thirty days of receipt.
