What is the Hight Health Atlanta Hypertension Initiative program?
The Hight Health Atlanta Hypertension Initiative is a blood pressure self-monitoring program that aims to engage and educate individuals, within the Atlanta metro community, on hypertension management. This 3-month program is free to participate and is open to those living in metro Atlanta.
How do you benefit from the Hight Health Atlanta Hypertension Initiative program?
- Lower or improve your blood pressure
- Reduce your risk for heart attack, stroke, or heart disease
- 1 - on - 1 support, in-person sessions, group support
- Access to nutrition classes and hypertension events
- Health-related giveaways, prizes, and more
Qualifications to enroll in the HHA program:
- Be at least 18 years old or older
- Been diagnosed with high blood pressure and on antihypertensive medication
- Have elevated blood pressure readings for 5 consecutive days (MUST track using American Heart Association blood pressure tracking form)
Requirements to Participate:
- Be willing to track your blood pressure on a daily basis.
- Be willing to attend virtual check-in sessions with a nurse to report on your blood pressure.
- Be willing to attend 3 nutrition classes.
- Be willing to attend the program graduation ceremony at the end of the program.
- Be willing to stay actively engaged for the three (3) months of the program.
