I am financially prepared to incur emergency expenses should my new cat/s need urgent care.
I agree to provide annual exams, vaccinations and any additional veterinary care for my new cat/s, by a qualified veterinarian, as needed.

By submitting this adoption application, I acknowledge that the information on this form is true and correct. I agree to all provisions indicated on this form. I understand that any misrepresentation of fact may result in Valley Cats, Inc. refusing adoption privileges to me. If my request for adoption is approved and later Valley Cats, Inc. discovers that the information is not true or correct, this application becomes null and void, and this constitutes a breach of contract. Because of my breach of contract, Valley Cats, Inc. reserves the right to remove the adopted cat/s from my home, and I will be held responsible for any associated costs incurred as part of said reclamation process. I understand that in order to ensure the best homes for its rescued cats, Valley Cats, Inc. reserves the right to deny any adoption application for any reason.
