The Glen Ellyn Aikido Club (“GEAC”), its members, Instructors, guest instructors, or anyone else, are unwilling to have people practice Aikido on its premises if the GEAC, its members, Instructors or visiting instructors, or anyone else, might be sued for injuries that may happen to anyone while practicing in the GEAC at the College of DuPage, Glen Ellyn, IL 60137 (COD) or for injuries that could occur, for any reason, anywhere on the premises or on the way to or from the GEAC/COD.
Therefore, we ask, in consideration and exchange for allowing you to practice at the GEAC/COD and other good and valuable consideration, the legal sufficiency of which is hereby acknowledged, on this date that you sign this document, and by doing so give up any rights you may have to sue or hold the GEAC/COD, its members, Instructors or guest instructors, or anyone else, liable for any injuries which may occur to you caused by negligence or any other reason, while anywhere on the GEAC/COD premises, including while on the mat and including while on the way to or from the GEAC/COD.
I, the undersigned, (or parents of a minor participant), have carefully read this document and understand it, and in consideration and exchange for practicing Aikido at the GEAC/COD and in further consideration as more fully stated above, give up any rights I may have to sue or hold liable the GEAC/COD, its members, Instructors or guest instructors or anyone else, for any injuries that may occur to me while practicing Aikido here or while anywhere on the GEAC/COD premises or while going to or from the GEAC/COD.
I fully acknowledge that participation in any martial arts program carries some risk of accident, injury, disability, or death. These risks may be caused by the actions, inaction or negligence of the participant, other participants, or instructor, including but not limited to GEAC/COD, there may also be other risks unknown to us or not reasonably foreseeable at this time.
I accept such risks and assume full responsibility for losses, damages, and possible injuries, disability or death, however, caused and whether caused in whole or in part by practicing in martial arts with the GEAC/COD.
I, the undersigned, have carefully read this document and understand it and in consideration and exchange for practicing Aikido at GEAC/COD on September 20-22, 2024, and in further consideration as more fully stated above, give up any rights I may have to sue or hold liable and hereby release, waive, discharge and covenant not to sue GEAC/COD, its instructors, officers, members, participants or guests or anyone else, in all martial arts activates or events related to the GEAC, and release from all liability to the undersigned, executor, heirs or kin, from any and all claims, demands, losses or damages on account of any injuries that may occur to me while practicing Aikido here or while anywhere on GEAC/COD premises or while going to or from GEAC/COD, death or damages to participants or property, related to events caused or alleged to be caused in whole or in part by the negligence of the GEAC/COD, its instructors, officers, members, participants or otherwise.
