The MBK will contact you within 2 weeks, if your child is shortlisted, to arrange for an enrolment interview and it is compulsory that your child is present.

During the interview and upon confirmation of registration, these are the necessary documents and photographs to be submitted.

(Note: For a foreign child with no Dependant Pass, Long Term Visit Pass or Immigration Exemption Order, an approved Student Pass is required before admission into the Kindergarten.)

4) CDA letter or card (if use CDA for fees)

5) Sibling's birth certificate (if use sibling's CDA)

 

B: Documents (copy) required for submission:

1) Child: Health Booklet (for birth order, developmental & immunization records / certificate)

2) Statutory Declaration of Marital Status (if any)

3) Letter of Authorization (if any, for guardianship)

4) Letter from lawyer firm, if in process of divorce

5) Divorce & Custody Papers with clause stating who has ‘care & control’ of child from previous marriage.

I understand that the MBK does not receive any form of government subsidy to defray the monthly school fees.

I understand that submission of this form does not confirm the enrolment of my child.

I understand that the MBK will contact me within 2 weeks if my child is shortlisted and will arrange for an enrolment interview which my child must be present.

I understand that I must produce all the required documents for verification during the interview.

I also understand that my interview will be rescheduled if I did not bring my child along.
