Reason for excursion/educational outcomes: Our children love to play and be active by climbing trees, doing obstacle courses, running around and playing on the playground so why not take them to and adventure playground for them to jump, play and explore.

Proposed activities to be undertaken: Kids World Playland is always a favourite for fun filled adventure! Featuring a fully air-conditioned centre with a GIANT inflatable slide, cannon ball battle zone, 3 storied climbing structures and a designated under 5’s area for little ones.

·I give permission for my child to attend the above excursion provided they meet the behaviour expectations of our service and this decision will at the discretion of the Co Ordinator.

·My child is healthy and physically able participate in the excursion outlined above.

·I agree that my child will be on a bus with a fitted seatbelt that he/she must wear.

·I understand any medications that are required to be administered on the day of the excursion will require a medication form to be completed and given along with the medication to the ‘responsible person in charge’.

·I understand that a risk assessment has been prepared and is available at the centre as per your request.
