Supplement pickup (Bay Area): If you’re local to the Bay Area, you may pick up your supplements at Tao to Wellness in Berkeley. Pick up windows are:

April 4th 9am-12pm

If you'd rather not pick up, we can certainly ship it to you. Just add the shipping cost ($18) and we'll send it out!

Shipping (outside the Bay Area): If you’re not local, supplements will be shipped to you. Please add $18 for shipping when submitting payment.

CONSENT AND LIABILITY

IN CONSIDERATION of being permitted to participate in any way with the services of Christina Martin and Tao to Wellness Acupuncture Inc.

I ACKNOWLEDGE, agree, and represent that I understand the nature of the Tao to Wellness Eliminate and Detox Program and that I am qualified and in good health to participate. I further agree and warrant that if at any time I feel unwell, I will immediately discontinue further participation in the program. I further agree and acknowledge that Christina Martin and/or Tao to Wellness Acupuncture Inc. does not warrant or guarantee results. I also understand that a physician’s approval is highly recommended prior to participating in any program.

I HEREBY WAIVE, RELEASE AND DISCHARGE Christina Martin and Tao to Wellness Acupuncture Inc., from negligence or fault of the persons released, for my death, disability, personal injury, property damages, or actions of any kind. I fully accept and assume all risks and all responsibility for losses, costs, and damages that I may incur as a result of my participation in this program.

I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Christina Martin and Tao to Wellness Acupuncture Inc. and their respective sponsors, vendors, advertisers.

I HEREBY RELEASE Christina Martin and Tao to Wellness Acupuncture Inc. from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the administration of the services provided.

I ACKNOWLEDGE and represent that I am twenty one (21) years of age or older, that I have read and understand the contents of this document, and that no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made.

I also agree, for myself and my successors, that the above representations are not mere recitals and that they are binding.
