- Any marketing brochures, program information and documents related to the proposed fundraiser program
- Copy of the parent permission slip, which includes the intent to take/use any team, individual, or group photo and any personally identifiable information; The Canyons District media release does not cover students participating in online fundraising
- Your company's written contract (if applicable). The contract must be submitted with the application as part of the review process
- Disclosure of online security procedures for student information
- Disclosure of the fee structure and details about different levels of service (if applicable)
- Disclosure of how the accounting process happens at the school level
