I understand that my child will be participating in outdoor activities during "Sensory Sundays" and thus, I acknowledge my responsibility to dress my child appropriately for the weather conditions. I am aware that outdoor play is an integral part of the program and that there is a possibility of my child getting messy. I agree to provide suitable clothing that will allow my child to comfortably and safely engage in these activities.*

I understand
By signing this application, I hereby give consent for "the participant" named herein to be photographed, videotaped, or otherwise recorded during their participation in Asibara Forests "Sensory Sundays". These recordings may be used by Asibara Forest for promotional, educational, or informational purposes.

I also acknowledge the risks associated with participation in the program and agree to release Asibara Forest, its staff, and affiliates from any liability for accidental injury or illness that may occur.

I accept full responsibility for any medical expenses that may arise from the participant's involvement in the program.
