I hereby agree to accept a position as a volunteer for The People's Kitchen of Philadelphia, and I acknowledge I have read and accept these conditions. 

This Release and Waiver of Liability executed on:

As a volunteer, I release and hold harmless The People’s Kitchen of Philadelphia and their successors from any and all claims, costs, suits, actions, judgments or expenses upon any damage, loss or injury to me or to my property which may arise from this volunteer event.

I acknowledge that I am fully aware of any and all risks posed by these volunteer activities and that I have no medical condition that prevents me from engaging in them.

I also acknowledge that I have read the People's Kitchen's Food Safety Guidelines and Pro-Community Guidelines and will do my best to uphold these standards. 

I also give permission to be photographed by project partners or the media for use in printed materials, through the internet or through other media outlets.

In typing my name below, I acknowledge that this is my digital signature, and I have read and understood this volunteer agreement.

If under the age of 18 you must have a parent or legal guardian sign this waiver before you may participate in any activities at or for The People's Kitchen of Philadelphia. 

As the guardian of the volunteer, you acknowlege that your name typed into the signature box below means you have read and understand this document.
