2024-25 Player Equipment Package Sizing Form

Each player contract includes an equipment package. To receive the correct package, it is important that you select the correct team from the drop-down menu below.

Lastly, you must complete your first payment to place your order, and you will need to complete your second payment to receive your equipment. There are no exceptions.
