To submit a process service request, please fill out the provided form. Upon submission, you will receive an invoice and further contact if necessary. A confirmation email will follow the receipt of your job details. By submitting the form and settling the invoice, you consent to a maximum of three service attempts at a single address. Additional attempts or different addresses will incur further charges. While the success of service is not guaranteed, supplying comprehensive information and photos, if available, can increase the likelihood of success. Customers should verify pricing prior to submission. Bona Fide Investigations Inc. values your patronage. To ensure transparency and a clear understanding of our operations, please carefully review the terms and conditions outlined below. Submission of a service request indicates your acceptance and agreement to these terms. All requests must be for lawful purposes. Bona Fide Investigations Inc. disclaims all liability for any claims, damages, or legal consequences arising from the nature of the request. Success in service is not guaranteed. All requests are billable upon submission. Service will commence only after payment has been arranged. Payments are accepted via Visa, Mastercard, Discover, and American Express. A 3% processing fee applies to invoices paid with credit card and will appear on the invoice at time f billing unless an ACH is provided. Should the address prove invalid, service will cease, and alternative options will be discussed. Upon successful service, an affidavit will be issued. Clients are responsible for filing with the court unless the affidavit filing add-on is purchased. Bona Fide Investigations Inc. reserves the right to refuse service for any lawful reason. Cancellations must be submitted in writing via email. Please note, no refunds will be issued once a request is submitted. A $50 collection fee will be added to all past-due accounts over 30 days. Additionally, interest will accrue at the applicable annual rate. By submitting your request, you confirm your agreement to these terms and acknowledge our privacy policy.
