Providing Access and Transforming Health (PATH) Success Story Submission Form

As community-based organizations, Medi-Cal providers, tribes, local government agencies, and others continue to participate in the PATH initiative, the Department of Health Care Services (DHCS) looks forward to showcasing their firsthand accounts of providing successful Enhanced Care Management (ECM) and Community Supports to the members they serve.

PATH stakeholders are strongly encouraged to complete the form below and provide their testimonial on how PATH has helped their organization achieve its Medi-Cal goals and strengthen services for members.

Success stories will be published to the PATHways to Success web portal and be featured on DHCS social media channels. Success story submissions will serve to promote your organization's commitment to transforming Medi-Cal and inspire and motivate other eligible organizations to access available PATH funding.

By submitting your organization's success story, you are consenting to having your story published to the PATHways to Success website and other DHCS public-facing channels, including social media. Prior to publication, a member of the PATH third-party administrator (TPA) team will contact you with the final story version for review.
