At Doodles By Faith, our goal is to help you find the perfect puppy to match your family and lifestyle. By filling out this application, you’re giving us the information we need to ensure a good fit for both your household and your potential new puppy.

Here’s how it works:

1. Complete the application below, providing as much detail as possible.

2. Once submitted, we will review your application and contact you via email within 72 business hours with the next steps, pending approval or denial.

Important Notes:

• Submitting this application does not commit you to purchasing a puppy, nor does it guarantee availability.

• This is a tool to help both you and us determine a good potential match for your family.

• If approved, you will be notified once a pregnancy is confirmed. You can secure your spot on the waiting list by submitting a $500.00 reservation fee. (Puppy picks are done at about 6 weeks in order of reservation).

A successful adoption depends on selecting the right dog and understanding its care requirements. Your thoughtful and complete answers will help us ensure this process is a success. Thank you for your interest!

I understand that completing this application does not guarantee I will be allowed to purchase a dog, and that the interest form is a tool to help Doodles by Faith determine if this dog and my family are a good fit for each other. I also certify that I have never been convicted of animal cruelty or neglect.

If I am approved to purchase a dog, I agree to pay a reservation fee of $500 and pay the remaining balance in cash at the time of pick up. If puppy is not being picked up in person, balance is due at time of puppy picks which is at about 6weeks. I agree to sign a purchase contract at the time of purchase or otherwise noted.

Do you understand all of our puppies are sold in a STRICT SPAY/NEUTER contract using Spaysecure, unless discussed otherwise. Breeding your dog without consent will result in a fine of $10,000 for breach of contract, plus a fine of $5,000 for each puppy produced.
