2024-2025 Corporation Amendment Form
Directions: Please submit ALL of the school changes within your corporation as well as any school improvement baseline reset requests, if applicable, at the same time. Make sure you click on the “Save” button after entering data in any section where applicable before submitting. Please send questions to schoolaccountability@doe.in.gov. Please note: nonpublic schools registering for a new school number should complete the Nonpublic School Number Request form found here.
Submissions are only accepted from April 1, 2024 - August 30, 2024, for the 2024 school year. Submissions outside this window or for future school years will not be accepted. In order to ensure requests are processed in a timely fashion, please make sure any documentation submitted clearly indicates approval for the requested changes. Vague or unclear documentation may delay request(s) processing time.
