This form is ONLY for CE certificates that were issued directly by the NASW-Michigan Chapter. The only CE certificates the NASW-Michigan Chapter issues themselves are for the following:
- NASW-Michigan Chapter events/trainings
- NASW-Michigan Chapter Book Club
DO NOT submit this form if you are seeking a CE certificate reprint from any of the following locations (please contact the course provider directly to request a CE certificate):
- Michigan Social Work CE Collaborative
- Events approved/administered by the NASW National office
- Events approved/administered by other NASW chapters
- Any other CE provider

There is a $20.00 fee for a CE certificate duplication request. This fee covers the time it takes for a member of our staff to verify attendance and recreate a duplicate certificate.

Once submitted, there are no refunds under any circumstances. This includes situations where a request cannot be fulfilled by our team, such as a CE certificate that was not issued by the NASW-Michigan Chapter.
I acknowledge that there is a $20.00 non-refundable fee for this CE duplication request. I also acknowledge that this fee will not be refunded even in situations where the NASW-Michigan Chapter cannot fulfill a request.*
