Campus Multimedia - 2025 Planet Fitness Franchisee High School Enrollment Form

Planet Fitness Corporate has negotiated a discounted rate of $90 per school.

Each franchise group will complete an agreement and payment terms are 50% due upon enrollment and 50% due on June 1, 2025.
