PEEK PARK PLAYGROUND PROJECT CONSTRUCTION
VOLUNTEER RELEASE AND WAIVER OF LIABILITY
In consideration of voluntarily participating in the construction of Peek Park Playground on the City of Cedartown's property at Peek Park, Cedartown, GA (hereinafter referred to as the "construction project") I, the undersigned, for myself, my heirs, executors, administrators and assigns, hereby waive and release any and all claims for damages for wrongful death, personal injury, loss of property or property damage I may have, or that may subsequently accrue to me, as a result of my participation in the construction project. This release and waiver is granted, without limitation in favor of their agents, and their respective officials, members, employees, representatives, agents, directors, sponsors, and organizers, and other participants in the construction project. I acknowledge that my participation is voluntary and done at my own risk, and I voluntarily assume all risk of loss, damage or injury that may be sustained while participating in the construction project.
I have read and understood everything written above, and I voluntarily sign this waive and release. (If the participant is under 18 years of age, the signature of a parent or guardian of the volunteer is required in addition to that of the volunteer.)
