Parent information

We submit your DOB and NI number to the local authority so they can check whether your child is eligible to receive a funding top-up called Early Years Pupil Premium.

Your child's eligibility

In this section, tell us what type of funding your child has and how many hours.

 

If you use hours at another nursery, you need to know which setting is using which hours – so please check before filling in this form.

** IMPORTANT NOTE **

If your child attends another nursery – DO NOT put the total hours your child is eligible for. Please enter the number of hours available to Into The Woods.

Subtract any hours used at the other setting.

Put zero for any funding types that you do not have.

Setting and attendance detail

– Please indicate how many funded hours per day are used at Into The Woods and, if applicable, your child's other nursery. 

– If your child only attends Into The Woods, put zero in the second setting boxes.

– Into The Woods days are 6.5 hours long.

Disability Access Fund

If your child is 3 or 4, is receiving child Disability Living Allowance and is receiving the free entitlement, they are eligible for the Disability Access Fund (DAF).  DAF is paid to your child’s early years provider.  The purpose of DAF is to support providers to make reasonable adjustments and build the capacity of their setting to support children with disabilities.

Early Years Pupil Premium (EYPP)

Early Years Pupil Premium (EYPP) is paid to childcare providers to provide extra support for your child. EYPP can be used to improve teaching and learning facilities and resources to impact positively on your child’s progress and development. Additional charges

Forest School Provision Charge – £40.50 per Forest School Day

Into The Woods charges a Forest School Provision Charge for all Forest School days – for main nursery and Forest School Clubs.

This charge is to cover the increased costs associated with delivering a Forest School type provision in public woodland – the additional staff training and qualifications, and higher staff ratios, in particular.

By submitting this form you are opting to pay the FSP charge for all Forest School days – ie, days when we are in the woods and parkland of Highgate Wood, Queen's Wood and the Kenwood Estate. 

Terms and conditions of receiving a funded place at Into The Woods

1. If a parent proposes to use a 30 hour code in order to receive a funded place, it is the parent's responsibility to ensure that the code is/will be eligible for the term in question. Into The Woods will not ascertain a code's elibility in advance of using it to make the funding claim (ie before the term starts), at which time it will be too late to reconfirm the code if it is not eligible.

2. For any given term:
– if a proposed code is not eligible, OR
–  Into The Woods cannot claim the number of hours stated by the parent, for whatever reason,
the parent agrees to pay the full fees at the full non-funded rate for the full term in question (ie both half terms).

3. Parents agree to inform us of any changes to the funded hours their child has available for use at Into The Woods.

4. If a parent terminates their child's place at Into The Woods and the child does not attend their Into The Woods sessions for the full notice period, the parent agrees to pay the full non-funded fees (session charge plus Forest School Provision charge) for the period they do not attend.  

5. Parents agree to provide parent declaration forms as requested by Into The Woods, and to pay the full non-funded fees if the form is not returned to Into The Woods by the deadline given. 

6. Parents of school age children (eligible to attend Reception class) agree to inform Into The Woods that funding is no longer available for their child if their child is (or will) also attend primary school at the same time as Into The Woods. (This is because funding will not be paid by some local authorities for children who are also in Reception.)

In signing below you confirm that the information you have provided is accurate and true. You understand and agree to the conditions set out in this document and authorise Into The Woods outdoor nursery to claim nursery entitlement funding as agreed above on behalf of your child.

Six weeks’ notice period for nursery entitlement funding

You understand that, once you have taken up a funded place at Into The Woods, if you wish to leave you will have to give 6 weeks’ notice before you can take up your entitlement at a new childcare provider.

Local authority privacy statement

In collecting your data for the purposes of checking your eligibility for the 2 year old, 3 and 4 year old universal, extended and enhanced free entitlements, Early Years Pupil Premium (EYPP) or Disability Access Fund (DAF), the local authority (Camden for Kenwood places, Haringey for Highgate places) is exercising the function of a government department. The local authority is authorised to collect this data pursuant to Section 13 of the Childcare Act 2006.

The Data protection Act 2018 (the Act) puts in place certain safeguards regarding the use of personal data by organisations, including the Department for Education, local authorities, schools and other early education providers. The Act gives rights to those about whom data is held (known as data subjects), such as pupils, their parents and teachers. This includes:

-The right to know the types of data being held
-Why it is being held; and
-To whom it may be disclosed

Should you have any concerns relating to how your information is being or will be used, please contact the relevant local authority (see above). 

Please note that information about whether a child is in receipt of Disability Living Allowance is, under the Act, Special Category Data which should be handled appropriately.  Providers are asked to pay particular note to advice from the Information Commissioner’s Office on holding personal data including sensitive personal data available at:
