This includes up to 2 filters per unit per visit. This service includes your Spring season service for A/C side of your RTU only.

Authorization to enter into contract.

I understand that by e-signing this document I am agreeing to the terms and conditions of this contract as well as receipt of Service Club Membership flyer explaining services rendered for scheduled billing; once signed this will be a legally binding agreement for 12 calendar months from the date of this contract. As such, I understand the quarterly and yearly billing costs as well as the services rendered under this contract. Community Mechanical Services, llc. will provide HVAC Service checklists by way of PDF document of services rendered based upon Service Club Membership. I promise to pay in a scheduled net 30 from services rendered and give permission to take legal action if payment is not fulfilled within 30 days of service completion. Finally, I have waived my rights to combat any service pricing as agreed upon within this contract for services rendered within the Service Club Membership and understand all benefits and have asked all pertinent questions relating to this agreement, scheduled billing, and services in return for scheduled billing.
