WHEREAS, Sweetgrass Pharmacy is a licensed compounding pharmacy in the State of South Carolina; and
WHEREAS, Physician or Practitioner is licensed in the State of South Carolina; and
WHEREAS, Physician or Licensed Practitioner desires to obtain compounded medications for office-use only; and
WHEREAS, Sweetgrass Pharmacy desires to provide compounded medications to Physician or Practitioner in accordance with South Carolina Board of Pharmacy Policies and USP Standards.
NOW, THEREFORE, in consideration of the mutual covenants contained herein, the parties agree as follows:
1. Services
1.1 Sweetgrass Pharmacy agrees to provide compounded medications to Physician or Licensed Practitioner for office-use only, in accordance with written, verbal, or electronic orders.
1.2 Sweetgrass Pharmacy agrees to make compounds in accordance with all USP Standards, including proper labeling with lot numbers and expiration dates.
2. Ordering
2.1 Physician or Licensed Practitioner will submit written, verbal or electronic orders for compounded medications specifying: * Physician name and DEA number * Product name, strength, and dosage form * Quantity * Any specific instructions
2.2 Sweetgrass Pharmacy will confirm the order's accuracy and compliance.
3. Records and Communication
3.1 Sweetgrass Pharmacy will maintain a record of all compounded medications dispensed to Physician or Licensed Practitioner, including: * Physician name * Product details * Lot number and expiration date
3.2 Physician or Licensed Practitioner agrees to maintain records of all compounded medications, including: * Lot numbers and expiration dates on patient records/charts * Usage information for office-use only * Documentation of proper disposal of expired or unused medications
4. Topical Anesthetics
4.1 Physician or Licensed Practitioner agrees to understand the risk of toxicity of topical anesthetics, albeit rare. The smallest effective amount of the medication should be applied and allowed sufficient time to take effect. Patients have the right to be informed about the potential risks and benefits of topical anesthetics before consenting to their use, and the obligation to disclose any allergies or medical conditions that might affect their safety.
