This four-week express program empowers participants to manage conditions like high blood pressure, diabetes, and chronic kidney disease through interactive workshops, fitness demonstrations, and educational sessions led by healthcare professionals.  

Participants will gain practical tools for meal planning, stress management, and physical activity to improve overall well-being.

Authorization to Use Photographs and/or Audio-Visual

I hereby authorize The Links, Incorporated to use, reproduce, and/or publish photographs and/or video that may pertain to me—including my image, likeness and/or voice, without compensation. I understand that this material may be usedin various publications, news releases, social media,electronically distribution, recruitment materials, broadcast public service advertising (PSAs) or for other related endeavors.

This material may also appear on The Links, Incorporated and affiliates internet web page. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, The Links, Incorporated may publish materials, use my name, photograph, and/or make reference to me in any manner that the organization deems appropriate in order to promote/publicize service opportunities.

Additionally, I waive any rights to royalties or other compensation arising from or related to the use of my image or recording. I understand that signing this authorization signifies my permission that photographic or video records of me may be utilized as listed above.
