That the media containing the subject in this consent shall be held by Quality Sound Voice Lessons/Angela Thomas.

That my information herein, as well as the media to be shared to the recipient shall not be released to any other party without my written consent;

That I hereby agree that such media, where the subject is included, shall belong to the organization for their use;

I hereby agree to waive my rights to any claims to the media and their use by the above stated organization.
