All requests require supporting supplemental documentation to be uploaded by the student. Failure to upload satisfactory supporting documentation may result in the denial of this request.
The Associate Dean of Students reserves the right to retroactively deny a request that was initially approved or tentatively approved if the student fails to submit supporting documentation promptly.
Students must upload supporting documentation for any of the categories within this Exam Conflict Request Form. This supporting documentation may include, but not limited to, the following:
- A letter from the medical professional who is performing the procedure;
- A copy of the death certificate of the member who has passed;
- An email receipt from COVIDCONFIDENTIAL@udc.edu after emailing them your positive test results;
- A copy of the program of service;
- A police report;
- A copy of the exam schedule with the student's final exams highlighted; or
- Documentation that shows the extenuating circumstance preventing the student from being present.
Please Note: These supporting documents cannot come from the student's family, partner, friend, coworker, and the like. Instead, it must be provided by the student's direct supervisor, a clergy member overseeing the service/ceremony, work authorization, etc. As a general rule, work-related requests and jury summons are not granted.
This documentation must identify the author and their credentials. And, it must indicate that the conflict is the same date and time that the exam was originally scheduled to be administered.
Students must provide documentation to the Associate Dean of Students as soon as possible, generally within one week of submitting the request. Failure to do so will result in their exam conflict request as null and void. (i.e. it never happened). Please refer to the Student Handbook about exam administration.
An exam will not be deferred until the student receives final approval from the Associate Dean of Students, Yannick Brookes. Until the student receives a written approval from the Dean Brookes, students are expected to be present and prepared to sit for their exam, as best as possible, despite their circumstances. A student who does not sit for their exam bears the outcome if the Dean of Students does not grant the exam conflict request.
Students should adheres to the submission deadlines that are listed below. This request needs to be submitted before the exam conflict occurs. Furthermore, in order for the Exam Conflict Request to be officially considered, students must submit this form. Email and/or telephone requests will not considered.
Deadlines: For final exams, this form should be submitted at least 2 weeks before the final examination period begins. For midterms, this form should be submitted 3 days prior to their exam date. Emergency situations will be managed on a case by case bases.
Deferred Exam Window: If your request is approved, students must have taken their deferred exam before the semester officially ends for final exmas. For midterms, students must sit for their deferred midterm within 10 business days from the date of the initial exam.
