Pickleball Liability Waiver and Release Form
Release of Liability
In consideration of being permitted to participate in pickleball activities organized by the Jones County Parks and Recreation Department (the “Department”), I, the undersigned, for myself, my heirs, personal representatives, or assigns, hereby agree as follows:
1. Assumption of Risk:
I understand and acknowledge that participating in pickleball involves certain inherent risks, including but not limited to slips, trips, falls, collisions, and injuries caused by equipment or other participants. I voluntarily assume all risks of injury or property damage associated with participation in these activities.
2. Release and Waiver:
I hereby release, waive, and discharge the Department, its officers, employees, agents, volunteers, and representatives from any and all claims, liabilities, actions, or causes of action arising from any injury, death, or property damage that occurs during or as a result of my participation in pickleball activities.
3. Indemnification:
I agree to indemnify and hold harmless the Department from any claims, damages, or expenses (including attorney’s fees) that arise from my actions or omissions while participating in pickleball activities.
4. Medical Treatment:
I consent to any emergency medical treatment that may be necessary during my participation. I understand that I am responsible for any medical expenses incurred.
5. Photo Release:
I grant the Department permission to use any photographs or videos taken during pickleball events for promotional or educational purposes.
6. Governing Law:
This agreement shall be governed by the laws of the state of Georgia
