WEMSA Cloud - Data Storage Center Onboarding

Start with this form to get set up with access to the Wisconsin EMS Association's Data Storage Center ("WEMSA Cloud") for your organization. WEMSA is partnered with FEMA and WEM to support EMS departments throughout Wisconsin, as well as extending support to organizations that support EMS.

Through grant funding, the WEMSA Cloud is currently FREE for participating organizations. Your cloud storage is only accessible by your department administrator or individuals you designate. WEMSA Staff or affiliated organizations do not have access to what your store on the WEMSA Cloud unless you file a support ticket that requires us to access data stored or if needed for ensuring proper data redundancy.

This form begins the process of setting up a FREE new account. After this form is submitted, there will be 2 additional electronic forms that will need to be electronically signed, in order for us to setup your cloud storage account for your organization. If these forms aren't completed, it will not be possible for our team to set up your organization.

ELECTRONIC AGREEMENTS

A Business Associate Agreement (BAA) is a standard form that allows us to store/ handle your organizations data that may be HIPAA sensitive, establishing a responsibility of our organization in handling protected health information (PHI) on behalf of your organization.

The other form is a Data Storage Terms of Use Agreement that requires agreement. These terms are similar to other cloud data storage providers (Google, Microsoft, etc.) that outline conditions for storing and accessing data on our data servers. These terms also cover aspects of data ownership, privacy, security, acceptable use policies, and provider rights.
