I acknowledge that I am voluntarily attending and participating in the “A Tour of Wellness” health fair. The Wellness Coalition shall not be liable for any injuries or medical related issues. My participation in all activities including physical activities is strictly voluntary. I also understand that there will be filming and photos taken at this event that may be seen by the public.
