First thing is to select Event
and then the proper selections will be shown.

File names should look like this:

FirstnameLastname_USA_ (ex: John Doe would be JohnDoe_USA_ )
- For Eligibility Form: JohnDoe_USA_EF
- For Headshot: JohnDoe_USA_Picture
- For Passport: JohnDoe_USA_Passport
- For Biography: JohnDoe_USA_Bio
- For Medical Form: JohnDoe_USA_Med
- For SafeSport: JohnDoe_USA_Safe
For the Eligibility Form, your university must fill out Section 2 before you submit it, or you can include a "Current Enrollment Verification Certificate" or current transcript (does not have to be official transcript but must show your name and school name). In all cases, fill out Section 1 and submit the form with any option that you choose. Do not just send a transcript by itself.

If you already submitted the Headshot and/or Passport during registration, you DO NOT need to resubmit unless you received a notice to do so. If you are submitting Headshot photo remember that it must have a solid WHITE background.
